Includes one virtual or in-person visit per month (wellness check-in or acute visit if needed. Discounts on labs and other wellness services within our practice. Common visits include: Prescription refills, sports physicals, wellness exams, yearly physicals, uti symptoms, Covid/flu like symptoms, sinus infections, pink eye, rashes, cold sores, insect bites, gout attacks, nausea, vomiting, diarrhea, follow-up visits (for stable care).

    We do not treat gynecological complaints, chest pain, or shortness of breath. These services require a higher level of care. Please advise patient to call 911 or go to urgent care for those concerns.

    We do not bill insurance: all visits are sell-pay for vour convenience
