30 minute session (virtual or in-person). Package includes: a comprehensive consultation + full lab panel (cholesterol, liver/kidney function, thyroid, testosterone, estrogen, progesterone, and more). Follow-up visit included to review labs and design your individualized treatment plan.

Follow Up Sessions

15 minute appointment. Monthly Membership. Direct access to Erin through email/portal for healthcare & wellness questions or concerns. Includes one virtual or in-person visit per month (wellness check-in or acute visit if needed. Discounts on labs and other wellness services within our practice. Common visits include: Prescription refills, sports physicals, wellness exams, yearly physicals, uti symptoms, Covid/flu like symptoms, sinus infections, pink eye, rashes, cold sores, insect bites, gout attacks, nausea, vomiting, diarrhea, follow-up visits (for stable care). We do not treat gynecological complaints, chest pain, or shortness of breath. These services require a higher level of care. Please advise patient to call 911 or go to urgent care for those concerns.

Includes initial consult with Erin, lab draw to test for over 286 food antigens, follow up with Erin to review results and curate diet and lifestyle guidance to help eliminate symptoms related to adverse food reactions.

Includes initial consult with Erin, lab draw testing APOE genotype to test for late-onset Alzheimer's disease and follow up with Erin to discus findings.
