Evolve Concierge Care is a
membership-based service designed to give you convenient, personalized access to care when you need it most. With a simple monthly membership, you’ll have direct access to Erin through a secure portal or email for healthcare and wellness questions, no long waits or unnecessary appointments.

Each month includes one 15-minute virtual , whether it’s for a wellness check-in or an acute concern. Common visits include prescription refills, wellness exams, sports physicals, and treatment for minor illnesses like UTIs, cold and flu symptoms, sinus infections, rashes, and more. Members also receive discounted pricing on labs and other wellness services within the practice.

This service is ideal for managing routine and stable health needs in a streamlined, accessible way. Please note that we do not treat gynecological concerns, chest pain, or shortness of breath, as these require a higher level of care.

For your convenience, all services are self-pay and we do not bill insurance.

  • $150 / month

  • Common visits include: Prescription refills, sports physicals, wellness exams, yearly physicals, uti symptoms, Covid/flu like symptoms, sinus infections, pink eye, rashes, cold sores, insect bites, gout attacks, nausea, vomiting, diarrhea, and follow-up visits (for stable care).

  • We do not treat gynecological complaints, chest pain, or shortness of breath. These services require a higher level of care. Please advise patient to call 911 or go to urgent care for those concerns.

  • We do not bill insurance: all visits are sell-pay for vour convenience
